LIFEPlan

LIFEPlan is a Care Coordination Organization (CCO) formed in 2018 along with six other CCOs in New York State to meet the needs of people with intellectual and developmental disabilities (IDD). They collaborate with service providers through a Health Home model.

They have one goal–to connect people to the spectrum of services they need to live full, meaningful lives.

In addition to offering the services of trained and experienced Care Managers, LIFEPlan partners with community-based agencies with a long history of serving people with IDD to provide advocacy and recreational, educational, and vocational services. Always with a person-centered focus, we connect Members with services and supports through a network of highly-trained human service and healthcare specialists.
